A mental health organization in Upstate New York is looking to add a new Social Worker to join their growing team. In this role, the Social Worker will be for providing culturally competent and evidence-based treatment and support services in a recovery-oriented environment to individual with mental illness

About the Opportunity:

• Setting: Forensic Mental Health
• Patient population: Ages 17+

Responsibilities:

• Assisting in providing therapeutic services to patients in a primary therapist role to include individual therapy and the completion of mental health assessments
• Completing documentation (Core History, Suicide Assessments/risk assessments, Weekly/Monthly Progress Notes, Treatment Plans, Discharge Plans)
• Providing crisis intervention, service linkages, and discharge planning
• Developing treatment plans with individuals and treatment team; monitoring and evaluating progress; and modifying treatment plans as needed
• Determining discharge needs and appropriate community resources
• Establishing and maintaining relationships with community providers to ensure continuity of care
• Performing other duties, as needed

Qualifications:

• Bachelor's and/or Master's Degree in Social Work
• Prior experience working and providing services to the Mental Health population

Desired Skills:

• NYS Licensed Master Social Worker (LMSW) and/or Licensed Clinical Social Worker (LCSW)
• Prior experience working in a Corrections setting
