Job Title: Care Manager
Duration: 6 months Contract (Potential to Extend)
Location: Lee County
Shift Type: 8am-5pm CST
Pay rate : $38 - $42 per hour
Day to day responsibilities of this the role:
- A Care Manager is responsible for outreaching members on a scheduled frequency to assess their unresolved healthcare needs work through barriers or obstacles to those needs being met. This may involve outreach to providers, suppliers, vendors, or other internal departments in order to coordinate the member’s care.
- Care Managers are required to meet the contract requirements set forth by AHCA and DOH. At a minimum, means either monthly or quarterly phone contact with our members, completing annual needs assessment packages with them, and quarterly in-person visits with some of our members. Accurate documentation and outreach is monitored on a continuous basis by chart audits, caseload audits, and recorded call audits.
- New care managers are required to attend 4 weeks of virtual instructor led training with our training department. Their supervisor will spend some time with them reviewing basic orientation for the first two days, but then the training department’s schedule takes over.
Required Skills/Experience:
- 2 years of experience working with the pediatric population is required.
- Health Plan Experience
- Experience in Child welfare system, community mental health agencies, and trauma-informed practices. (Preferred)
- Bi-lingual (Preferred)
- Microsoft Office – Outlook, Word, Excel
Education Requirement:
- Requires a Master's degree in Behavioral Health or Social Work or a Degree from an Accredited School of Nursing and 2 – 4 years of related experience.
