Job Title: Home Health Physical Therapist • Days

Join our team as a dedicated Physical Therapist in a 13•week contract role. This position requires a flexible individual ready to work a 40•hour week, including potential weekends.

Responsibilities:

• Deliver home•based physical therapy to patients

• Handle a weekly caseload of 20•25 patients

• Ensure same•day service documentation

Requirements:

• Valid CA PT License

• BLS certification

• 2 years of relevant experience

• Proficiency in OASIS and Homecare Homebase

Additional Details:

• No on•call or employment gap over 1 year

• Pre•approved time off

• Weekly in•person office attendance

• Location: Petaluma, CA

• Orientation: 2•3 days

Home health experience and Oasis documentation are essential for this role.
