JOB DESCRIPTION:

The In-Home Services RN Manager is responsible for ensuring completion of initial and annual assessments of participants in their home environment to determine their individual needs and coordinate plans of care (POC). In addition to providing quality care, the In-Home Services Manager is responsible for the supervision of the home care staff and their functions. Physical attendance at a facility and/or participant home is essential to performing the job duties.

• Supervision & Administrative – 90%
• Participant Care – 10%
• Monday - Friday, 8:00am-5:00pm

REQUIRED:

• Current State Registered Nurses License
• Bachelor of Science in Nursing
• Experience in Management with an emphasis on geriatrics; home health management experience would be preferred
• 2+ years practicing as an RN with an emphasis in geriatrics
• Current First Aid and BLS certifications are required prior to hire. Acceptable vendors for certification are American Heart Association and Red Cross.
