Job Description
PT Registered Nurse Care Coordinator position for SLH. Responsible for ensuring quality patient outcomes and overcoming obstacles to delivery of care through interdisciplinary coordination and communication. This position coordinates the interdisciplinary team and partners with physicians to ensure patient treatment plan progress. The Care Coordinator will ensure that an appropriate plan of care is initiated and will monitor care progression to ensure quality outcomes and effective transitions of care. BSN Degree. BLS and current MO RN license are required. Prefer a minimum of one year in-patient RN experience. This position is 24 hours a week.
