*RESPONSIBILITIES:*
* Provide primary nursing care to residents with varying backgrounds and medical histories
* Assists with new admissions
* A Licensed Practical Nurse (LPN) will monitor and communicate changes of condition to provider
* Provide medical intervention as needed for emergent needs
* Communicate needs of resident to outside providers including but not limited to: Primary Care Provider, Home Health Agency, Pharmacy, etc.
* As a Licensed Practical Nurse (LPN) you would communicate resident condition to family, and or responsible party members
* Implement and follow Primary Care Providers orders
* Other Duties as Assigned

*Qualifications*
* Graduate of accredited LPN School of Nursing.
* Current valid LPN license in the state employed.
* Valid CPR certification.
* Ability to react decisively and quickly in emergency situations.
* Ability to communicate effectively with residents and their family members, and facility staff.
