We have a newly renovated Memory Care unit at Toledo, a newly renovated skilled wing at White Oak, and Sylvania is under new management.

*We are looking for Physical Therapists *to ensure that the client will function at their personal best. You will maximize the quality of life and movement potential within the spheres of rehabilitation. You will lead, guide, and direct the delivery of Physical Therapy within our buildings in accordance with local, state, and federal regulations, standards and established facility policies and procedures to provide appropriate care and services to residents. This position will require travel within 15 minutes of your home building.

*Responsibilities:*

Perform physical examinations of patients to diagnose and treat various conditions.

Provide quality, personalized and evidence-based care and proven interventions.

Promote residents’ healthy lifestyle by improving strength, flexibility, and coordination.

Provide accurate and timely treatment and medical documentation.

Evaluate effects of treatment plan; and monitor and communicate progress

Ensure that appropriate treatment is given to patients under the supervision of a physician or other medical professional.

Ensure that all necessary forms are completed and processed in accordance with established medical policies and procedures.

Knows and understands the purpose and requirements for the scope of work and supervision requirements as defined by the Ohio physical therapy practice act.

*Skills:*

Excellent interpersonal communication skills

Valid Ohio license to practice physical therapy

Graduate degree or greater in physical therapy

Experience working as an physical therapist (Preferred)

Knowledge of skilled nursing home regulations, procedures, laws, regulations and guidelines pertaining to long-term care (Preferred)
