*Job Description:*
As a Physical Therapist in a Skilled Nursing Facility, you will be responsible for providing high-quality physical therapy services to patients with a wide range of conditions. You’ll create and implement personalized treatment plans, evaluate patient progress, and collaborate with a multidisciplinary team to improve patient outcomes.

*Key Responsibilities:*
* Evaluate and assess patient conditions and needs
* Develop and implement individualized treatment plans
* Monitor and track patient progress, adjusting treatments as necessary
* Educate patients and their families on exercises, therapy techniques, and recovery goals
* Work closely with other healthcare professionals to provide comprehensive care
* Ensure compliance with facility policies, procedures, and regulations

*Job Requirements:*
* Valid Physical Therapist license
* Previous experience in a Skilled Nursing Facility (SNF) setting preferred
* Strong communication skills and ability to work within a team
* Ability to develop and implement effective treatment plans
* Compassionate, patient-centered approach to care
