Nation-wide primary care organization geared towards the geriatric population seeking a Medical Director to help build out a home care program in Philadelphia. This is a full time permanent position with a 4-6 week orientation/training period, stock options, extensive PTO, large CME stipend, malpractice coverage and 401k with match available from day one. Mon-Fri 8am-5pm hours, no weekends, evenings, holidays or on-call required

Job Function:

Oversee NPs (team of 10) that work on a care team with a scribe, nurse and social worker to provide care throughout Philadelphia
Expect to have 3 days "in the field" doing team huddles with care teams or home visits as needed for backup to the team, remaining days can be from home
Work in conjunction with senior leadership on scheduling and assessing quality and efficiency of the program (new to Philly but organization has successfully set up same system in other major US cities)
Expect understanding of managed care and value-based care program as goal is to keep patients out of the hospital by providing care at the home or triaging back to one of the organizations' care centers

Job Requirements:

Board Certification Required
PA State License Required
Training as Family Medicine, Internal Medicine or Geriatrician
Previous home care, nursing home or primary care experience required
