CLINICAL HOSPITAL PHARMACIST JOB DESCRIPTION:
The Clinical Pharmacist position is responsible for providing clinical and operational pharmacy services to the highest standard of pharmaceutical care principles for each patient within their area of responsibility in an acute hospital setting. The position is responsible for therapeutic assessment, interventions, and plans for individual patients as well as patient populations. The Pharmacist is a valued member of the Interdisciplinary Care Team and assumes responsibility for medication-related outcomes. The pharmacist aligns with the pharmacy enterprise’s vision of improving outcomes and bringing value to our patients.
SHIFT/SETTING:
Location: Mauston, WI
Schedule: Monday - Friday (5x8) shifts vary ( 8am-6pm or 9am-7pm); Every 3rd Saturday 8am-1pm
Setting: Acute/Hospital
Start Date: ASAP
CLINICAL HOSPITAL PHARMACIST RESPONSIBILITIES:
*Maintain current knowledge of drugs, their composition, actions, uses, toxic effect, dosage, availability and distribution.
* Ability to prepare sterile solutions.
* Familiarity with Federal and State laws regulating the profession of pharmacy and handling of drugs.
* Superior ability to work tactfully with patients, public, physicians, and other members of the healthcare team, and skillfully supervise daily workflow and personnel.
* Ability to demonstrate initiative, and problem identification and solving skills.
* Excellent written and verbal skills for teaching, presentations and persuasion.
* Knowledge of pathophysiology and disease states required to evaluate complex therapeutic issues and to monitor and advise on drug therapy.
* Familiar with unit dose, IV admixtures, computer skills, technical and clinical pharmacy practice
CLINICAL HOSPITAL PHARMACIST REQUIREMENTS:
*Unrestricted Wisconsin State Pharmacist License - REQUIRED
*AHA BLS/ACLS - REQUIRED
