A Brief Overview

Under the direction of the Director of Nursing/LTC Clinical Coordinator the LPN is responsible for the planning and coordination of direct resident care services. Serves as a core member of the interdisciplinary client care team. Has the necessary authority to assess, plan, evaluate and teach in patient situations. Carries out administrative responsibilities as delegated by Director of Nursing or the LTC Clinical Coordinator.

What you will do

  • Performs professional nursing care to residents, according to needs and is responsible for direct supervision of nursing activities of facility.
  • Conducts daily rounds and communicates to appropriate team members during the shift. Contacts providers, establishes good working relationship and ongoing information exchange.
  • Administers routine medications, documents and make decisions about the use of prn medications. Obtains specimens for lab tests as ordered.
  • Consistently & thoroughly completes documentation in a timely manner, including documentation of all medications and treatments administered and participates in the development of interdisciplinary plan of care.
  • Participates in interdisciplinary team meetings. Initiates, participates and coordinates additional meetings as necessary to address crisis matters and provide support for team members.
  • Maintains established departmental policies and procedures, objectives, quality improvement, environmental safety and infection control standards.
  • Initiates resident referrals, transfers and discharges.
  • Ensures that assigned CNAs/CMAs staff understand each residents’ plan of care. Assists in training and education of nursing and nursing support staff, participates in QAPI activities.
  • Maintains supplies and equipment in adequate numbers and working order following infection control policies.
  • Provides resident and family education by direct instruction, utilization of interdisciplinary team members or appropriate community resource.

Required Education, License/Certification, or Work Experience:

  • Licensed Practical Nurse (LPN) - Board of Nursing Active license in the state of practice Upon Hire
  • Minimum of 3- 5 years experience
