Job Requirements
- Primary Source Verification of professional license and/or certification, as required for the position ran within 30 days of start updated upon expiration.
- Temporary Licenses are NOT accepted – must have a valid permanent license.
Additional Details
- Proof of vaccine series or completion and approval of client medical or religious exemption form. Preferred, not required.
- Positive MMR titers ONLY from within the last 15 years.
- Completed within 1 year of start updated annually. Cannot be done on site.
- Will accept one of the following: (a) Proof of full Hepatitis B series (three (3) vaccines) completed (b) Must have at least two (2) vaccines from the series completed, or (c) signed declination, agency form acceptable.
- Positive Varicella titers ONLY from within the last 15 years.
- Proof of vaccination within past 10 years, declinations not accepted.
- 2-Step TB test with negative reads within the last 6 months. Either true PPD 2-step or QuantiFERON/TSPOT is acceptable.” This is an annual requirement.
