Job Description

We are looking for a compassionate, dedicated, and experienced Primary Care Physician (Internal Medicine or Family Medicine) to join our dynamic team. As a Primary Care Physician at Foundation Medical Partners, you will play a key role in diagnosing, treating, and providing preventive care for patients. Responsibilities:

  • Provide comprehensive primary care to patients of all ages (family medicine) or adults (internal medicine), including preventive care, diagnosis, treatment, and management of acute and chronic conditions.
  • Perform physical examinations, take medical histories, and order necessary tests to guide treatment plans.
  • Diagnose and treat various conditions, creating individualized care plans for each patient.
  • Collaborate closely with specialists across a broad range of disciplines within our connected care model to provide comprehensive, coordinated care.
  • Educate patients and their families on wellness, lifestyle modifications, and disease prevention.
  • Maintain accurate and up-to-date medical records in compliance with healthcare regulations.
  • Participate in community outreach programs and health education initiatives.
  • Foster a positive and supportive work environment, promoting teamwork, communication, and professional development.

Qualifications:

  • Medical degree (MD or DO) from an accredited medical school.
  • Board certification or eligibility in Internal Medicine or Family Medicine.
  • Current, unrestricted medical license in NH.
  • BLS/ACLS certification required.
  • Strong clinical skills with the ability to manage a wide variety of conditions.
  • Excellent communication skills and the ability to build strong relationships with patients and colleagues.
  • Commitment to providing high-quality, patient-centered care.
  • Ability to work collaboratively within a connected care model, integrating input from specialists to deliver comprehensive care.
  • Dedication to continuous learning and professional growth.
