  • Typical Assignment is 9 evaluations w/expectation of completing 8
  • Monday thru Friday with occasional Saturday

  • State License Requirements
  • Documentation of vaccination series completion OR documentation of declination to vaccine to be signed and dated by traveler
  • 1st PPD within 1 year, 2nd PPD within 30 days of start
  • Must not be expired. Must match address on file within technology
  • Valid 10 years from the administered date. No declinations or attestations accepted.
    Schedule Information
  • Monday thru Friday with occasional Saturday
    Unit Specific Information
  • Clinical only, to be completed within one year of start; updated annually for specialty.
    Additional Details
  • Documented proof of two MMR vaccines or Positive IGG Titer. No declinations accepted.
  • Required for all travelers. Preferred to be completed prior to start, can be completed onsite.
  • Flu season is October 1, 2024 through March 31, 2025. Exemptions/Declinations are accepted, but candidate will be required to wear a mask at all times.
