  • Shift: 5x8 Days 8:30a - 5:00p - with weekdays only and no weekends or call
  • HOME HEALTH EXP REQUIRED
  • FACILITY WILL REIMBURSE MILEAGE AT IRS STANDARD RATE
  • Experience required: 1 year

  • Skills Required: Home Health; Documentation - Assessment, Discharge, Evaluation, Long Term Goals, OASIS, Plan of Care, Reassessments, RUG Levels, Short Term Goals, Time Management
  • Skills Preferred: Assessment - Adaptive Devices, Assistive Devices, Body Mechanics, Identifying Environmental Barriers, Pain assessment, Strength testing; Equipment and Devices - Gait belt, Hospital bed, Sit to stand lift, Wheelchair Assessment; Orthopedics - Extremity Mobilization, Gait training, Hip fractures/injuries, Osteoarthritis, Spinal Conditions, Total hip replacement, Total knee replacement; Neurologic - Post Stroke Rehabilitation; Professional Skills and Knowledge - Home Exercise Program, Patient family education
  • REQUIRED: Knowledge of OASIS
  • EMR: Netsmart (McKesson)
  • FACILITY RADIUS RULE IS WAIVED FOR THIS POSITION
  • FLU VACCINE: Davis will consider medical exemptions. Travelers MUST provide a signed doctor’s note from their physician AND complete Davis’ medical exemption request form. Davis is NOT considering religious exemption requests
