50% Estherville, IA SNF
50% Jackson, MN SNF and HH in that area
Occasional Windom SNF coverage
Drive Time and Mileage paid between facilities and during HH travel
Skilled/Rehab and Home Health - Post Acute or Long Term residents and Outpatients
1 PTA to supervise
New Grads Welcomed Therapy Supervisor, Director, and Clinical Specialist support
Software - Netsmart for Home Health and Net Health for SNF - training available
Offering at least 13 weeks and option for longer term - 1 year
Flexibility in schedule for time off requested or to shift worked days: Normal is M-F could do Su-Th to have F off
HH is approx 6 clients per day and SNF Productivity expectations 85% (includes evaluations and group/concurrent time in full)
Guaranteed - 36 hours
