The Wound, Burn and Ostomy Nurse Practitioner is responsible for providing comprehensive wound care assessments and treatment plans for patients with acute and chronic wounds. Utilizing evidence-based practices, the practitioner conducts advanced wound care procedures while educating patients and healthcare staff about effective management strategies. This role emphasizes collaboration with healthcare professionals and maintaining compliance with regulatory standards in a mobile healthcare setting.

Nurse Practitioner wound Care - Abilene, TX

Job Type: Full Time

Shifts: Days

Unit: Mobile

Patient Ratio: 4-7 per day

Responsibilities:

  • Conduct thorough assessments of patients with acute and chronic wounds, including wound evaluation, measurement, and documentation.

  • Develop and implement individualized wound care treatment plans based on evidence-based practices and patient-specific needs.

  • Perform wound debridement, dressing changes, and other advanced wound care procedures using sterile techniques.

  • Educate patients, caregivers, and healthcare staff on wound management, prevention strategies, and self-care techniques.

  • Collaborate closely with physicians, nurses, therapists, and other healthcare professionals to optimize patient outcomes and continuity of care.

  • Utilize telehealth and mobile technology platforms effectively to support remote consultations and follow-up care.

  • Maintain accurate and timely electronic health records (EHR) documentation in compliance with regulatory standards and company policies.

  • Participate in interdisciplinary team meetings, case conferences, and continuing education activities to enhance clinical knowledge and skills.

Qualifications:

  • Master's degree or higher in Nursing from an accredited Nurse Practitioner program.

  • Current national certification as a Nurse Practitioner (NP) with specialization in wound , ostomy, and continence nursing (WOCN) preferred.

  • Active state licensure as a Nurse Practitioner and current certification in Advanced Practice Registered Nurse (APRN).

  • Minimum of 1 years of clinical experience in wound care management, preferably in home health, long-term care, or outpatient settings.

  • Proficiency in performing wound assessments, debridement, and advanced wound care procedures.

  • Strong clinical judgment and decision-making skills with the ability to work independently and as part of a collaborative team.

  • Excellent communication, interpersonal, and patient education skills.

  • Commitment to compassionate patient care, empathy, and cultural sensitivity.
