The Family Practice/Primary Care Nurse Practitioner provides high-quality primary care to homebound patients through home visits. This role requires strong clinical skills in managing older adults with complex health issues and collaboration with a healthcare team including medical assistants and social workers. The ideal candidate is passionate about serving under-resourced communities and has experience in home-based care, especially with Medicare populations.
- Board Certified Nurse Practitioner with experience
- Home Visit NPs deliver high-touch, high-quality primary care to our homebound patients. - This role is supported by ascribe/ ? medical assistant, nurse, social worker, and other resources.
- NPs make 6"7 visits daily in adefined territory and collaborate continuously with other team members for care coordination and all otherneeds.
- The ideal candidate will have a genuine passion for primary care and reaching under-resourced communities
- Strong clinical skills focused on older adults with complex disease states
- Experience in home-based care (as RN orAPN)
- 1+ years APN experience, preferably with aMedicare population
- Bilingual (English and Spanish) preferred, but
not required
