Responsibilities
- Monday Friday (8:00am 5:00pm)
- Evenings as needed
- Rotating Weekends (Saturday morning)
- Conducts mental health crisis assessments, formulates dispositions, and coordinates within primary medical services
- Assists with patients' stressors, coordinates prior authorizations, medication needs, and enhances follow-up care adherence
- Provides strength-based, patient-centered services aligned with clinical standards of practice.
Requirements
- Current and active PMHNP licensed in the state of Connecticut
- Bilingual (Spanish) required
