Located in Dzilth-Na-O-Dith-Hle, NM, approximately 30 miles south of Bloomfield, NM, the health center is under the administrative oversight of the Shiprock Service Unit. Established in 1984, DZHC serves the communities of Huerfano, Nageezi, and Counselor Chapters, as well as IHS beneficiaries from the urban towns of Bloomfield, Aztec, and Farmington, NM.
