In this dynamic role, you’ll split your time between Belle Plaine Specialty Care, a 46-bed skilled nursing facility, and Montezuma Specialty Care, a 41-bed skilled nursing facility. Your schedule will alternate each week, working two days at one facility and three days at the other, providing flexibility and variety.
As an MDS Coordinator, you will play a vital role in ensuring the accurate and timely completion of assessments and care planning, supporting the highest quality of care for our residents.

What You Need to Become an MDS Coordinator (RN) at Care Initiatives:
• A valid RN license in good standing within the state of Iowa, or the ability to activate one.
• Current, valid CPR certification.
• Previous MDS experience is preferred but not required.
