Job Summary:

Firesteel Healthcare Center is currently looking for a Resident Care Manager Nurse/MDS Assistant to oversee the proper administration of total care to our residents. Provides general nursing care to residents, and oversee the work of the Licensed Nurses and CNAs in our facility. The ideal candidate will also assist with the development and completion of the Resident Assessment Instrument (RAI), that is, the Minimum Data Set (MDS), Care Area Assessments (CAAs) and Care Plan in accordance with State and Federal requirements.

Essential Functions:

Obtains and administers prescribed medications and treatments in accordance with approved nursing techniques.
Observes resident, records significant conditions and reactions, and notifies supervisor, family, (responsible party) and physician of resident's change in condition and reaction to drugs, treatments, and significant incidents.
Takes vital signs to detect deviations from normal and assess condition of resident.
Administers appropriate disciplinary action for nursing staff non-compliance with state and federal regulations and Center policies.
Modifies resident care plan, care directives, Activities of Daily Living (ADL) and Aide flow sheets in conjunction with resident changes in condition.
Coordinate the IDT in timely completion of the assessments.
Utilize MDS Scheduler to plan and set MDS Assessment Reference Dates (ARDs).
Communicate ARD selection for specific MDSs to IDT.
Complete Care Area Assessments (CAAs) as required.
Complete care plan items relevant to current MDS and attend care plan meetings as assigned.
Monitor the overall documentation in the medical record to validate that it supports MDS coding.
Participate in Medicare utilization management as assigned.

Education, Licensure, and Experience:

Required: Graduate of an accredited school of nursing.
Required: Current LPN or RN licensure in the state of employment.
Required: Computer aptitude with the EMR for this position
Preferred: Two years’ experience working in a long-term care
Preferred: Experience with Residence Assessment Instrument (RAI) process and MDS certification
Preferred: Point Click Care (PCC) experience
Demonstrated knowledge of nursing process, particularly as it relates to geriatric residents, long-term care, and rehabilitation.
