Job Description
As a nursing professional, you have a calling to help others. We are looking for a licensed nurse who is detail-oriented and dynamic to be a PPS Skilled Case Manager for our facility. The PPS Skilled Case Manager oversees gathering of information for assessments and care plans. Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) are both encouraged to apply.
Responsibilities and Purpose
Oversee gathering of information for assessments and care plans.
Provide maximum flexibility in coordinating the payer's needs with patient outcomes.
Communicate effectively with the treatment team and influence the treatment approach, when needed.
Work cooperatively as a member of a team.
Desired Experience and Education
Current and unencumbered Oklahoma Registered Nurse (RN) or Licensed Practical Nurse (LPN) License.
Associate's Degree plus appropriate certification or BSN.
Charge Nurse Experience in Long-Term Care.
Basic computer skills, including Microsoft Office.
Minimum of 2 years MDS & LTC experience or Acute Case Management experience preferred.
Experience with MDS and care planning process preferred.
