Bayshore Residence and Rehabilitation Center is looking for an In-House PRN Occupational Therapist to become a part of the team!
The Occupational Therapist is responsible for assessing, planning, implementing, and evaluating individualized treatment plans for patients who have difficulty with daily living activities due to physical, mental, or emotional impairments. The OT works closely with the patient, family members, and other healthcare professionals to help the patient achieve their goals.
Other Responsibilities Include:
• Assessing referred patients that require rehabilitation services which includes providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions, and potential for rehabilitation.
• Establishing and administering a treatment program with specific goals determined according to the patient's capacity and tolerance under the direction of the Physician.
• Setting realistic and achievable goals for their patients, documenting and recording the patient's condition.
• Educating patients and families with regard to the plan of care.
• Monitor patients’ progress and adjust treatment plans as necessary to ensure optimal outcomes.
• Work collaboratively with other healthcare professionals, such as physicians, nurses, and physical therapists, to provide comprehensive care to patients.
• Maintain accurate and up-to-date patient records and documentation, including evaluations, progress notes, and discharge summaries.
• Stay current with industry trends and best practices, and participate in continuing education and professional development activities.
Qualifications/Requirements:
• Graduated from an accredited Occupational Therapy college program or university.
• Valid professional license and registration for the state of MN
• Experience with long-term care and/or geriatric care is preferred.
• Well-versed with EMR documentation evaluations, weekly progress notes, recertification, and discharge documentation.
• Ability to work independently and as part of a team.
• Collaborates with all disciplines to provide the best patient-centered treatment and care.
• Exceptional patient service skills and ability to demonstrate clinical excellence
• Ability to adhere to scheduled hours and demonstrate professionalism at all times
• Perform all other duties assigned by the Director of Rehabilitation Compliance with confidentiality & HIPAA
