Position Summary: Accountable for coordinating care across the continuum for an identified group of patients. Participate in the development of appropriate plans of care in conjunction with the physicians and other members of the health care team. Responsible for insuring and facilitation the achievement of quality, clinical, and cost outcomes, seeking approval for and arranging services and resources needed by the patient and family, intervening at key points for individual patients to meet defined patient outcomes.
Experience: Three or more years acute care nursing experience. Previous experience with case management systems or utilization review preferred.
Education: Current NJ - RN license. Bachelors degree in related field preferred.
Other Required License/Credentials/Skills: Excellent interpersonal skills. Working familiarity with personal computer. Familiarity with ICD-9 coding.
Contact With Others: Regular internal and/or external contact involving considerable tact, discretion and persuasion in order to obtain information and/or willing action or consent.
Supervision Given: None
Supervision Received: Works under general supervision in performing routine assignments, referring unusual or more difficult situations to immediate supervisor.
Bi-Weekly Hours: 80 hours per pay period / 8A-4:30P
