Job Requirements
- Valid for 2 years. Available for free at https://calcivilrights.ca.gov/shpt/ Covered in WAMC orientation packet when they come into facility under Essentials.
- Primary Source Verification of professional license, and/or certification, as required for the position. Ran within 30 days of start. Updated upon expiration.
- Driver's license, State ID, or Passport
- Upload "Essentials" Form and Post Test. Found in Document Library. Orientation paperwork is given to the Clinician on their first scheduled shift to complete on site.
- Please upload a notarized name affidavit for any name discrepancies between credentials.
- National Criminal Background Check within one year of start date. Exceptions are: FL / MI / TX: Must have background check completed before beginning assignment. AL / OH / NV: Agency employees assigned to Behavioral Health unit must have background check before beginning assignment. GA / NJ / OH: Agency employees assigned to Substance Abuse Program unit must have background check before beginning assignment.
