    Certification Details
  • COVID-19 Vaccine - Preferred, not required.
  • MMR - Must show a new positive titer after booster series.
  • Fit Test - Completed within 1 year of start updated annually.
  • Hepatitis B - Proof required or signed declination.
  • Positive PPD- Negative Chest X-Ray and Annual TB Questionnaire if positive.
  • Tdap - Proof of vaccination within past 10 years.

  • Replacement for ESS leaving Ahuja.
  • Must have ACLS/BLS
  • Must have min. 2 years of experience
    Job Requirements
  • BLS - American Heart Association only. Required for clinical positions.
  • ACLS - American Heart Association only.
  • License/Certificate Verification - Primary Source Verification required. Temporary Licenses are NOT accepted.
  • UHS Skills Checklist - Skill specific required to be complete within 1 year prior to start.
  • UHS Competency Exam - Competency Exams for the relevant department must be complete within 1 year prior to start.
  • UHS Core Competencies - To be complete within 1 year prior to start.
  • Physical/Health Statement - Physical Exam completed within one year of initial start date.
  • UHS Criminal Background Check Results - To be run upon initial hire.
  • UHS OIG Search - To be complete within 6 months prior to start.
  • UHS SAM/EPLS/GSA/OFAC - Supporting documentation required.
  • UHS Drug Screen - To be run upon initial hire.
    Additional Details
  • American Heart Association only. Required for clinical positions.
  • Primary Source Verification of professional license and/or certification, as required for the position ran within 30 days of start updated upon expiration.
  • Proof of vaccine series or completion and approval of client medical or religious exemption form.
