- Documentation of vaccination series completion OR documentation of declination to vaccine to be signed and dated by traveler
- 1st PPD within 1 year, 2nd PPD within 30 days of start
- Documented proof of two Varicella vaccines, serological evidence of immunity, or verification of a history of varicella or herpes zoster (shingles) by a healthcare provider
- Valid 10 years from the administered date
- Statement of good health or completed physical exam prior to first assignment, updated annually thereafter
