Salary and Benefits:

• Salary Range: $230,000 to $250,000 annually
• $10,000 Sign On Bonus
• Relocation Assistance Up to $5,000
• Vacation: 120 hours
• Paid Holidays
• Continuing Medical Education (CME): 40 hours, allowance of $2,500
• Medical, Dental, and Vision Insurance Employer Pays 80%
• Life Insurance: 1.5x the provider s salary
• Short-Term and Long-Term Disability
• Retirement Package 403(b)
• Employer Paid Malpractice Insurance

Responsibilities:

• 4-day work week, Monday to Thursday
• One Saturday per month from 8 AM to 1 PM, rotating among staff
• 36 hours of clinical work and 4 hours of admin a week

Requirements:

• Board-certified or Board-eligible Family Medicine Physician
• New Graduates Will Be Considered
• Compassionate, patient-centered approach in primary care
• Clear and Active Medical License in the State of Indiana
