Heritage Therapy provides high-quality physical, occupational, and speech therapies and rehabilitation at many of our communities.

We strive to help patients reach their maximum potential so they can live as independently as possible. Our experienced therapists work closely with patients and their caregivers to build customized rehabilitation programs tailored to each individual’s needs.

Rockport is a 125 bed assisted living facility and we will be providing outpatient therapy services. The therapy gym will be furnished with exercise equipment, bathroom, and shower to address simulated ADL tasks. We will be utilizing Net Health for documentation.

\*\*Duties:\*\*

- Complete evaluations/10th visits/discharges, and also co-sign notes for the COTA

- Educate patients and their families on treatment strategies, home exercises, and preventive measures.

\*\*Skills:\*\*

- Possess a deep understanding of anatomy and physiology to inform treatment decisions effectively.

- Strong interpersonal skills with the ability to communicate effectively with patients, families, and colleagues.

- Ability to thrive in a fast-paced environment while upholding high standards of patient care.

- Active state licensure as a Physical Therapist is required; eligibility for licensure will also be considered.
