*Summary:*

Provides physical therapy evaluations and treatments to appropriate patients to facilitate their return to prior level of functions as prescribed by physician. Includes the identification, prevention, remediation and rehabilitation of patients who have impairments, functional limitations, disabilities or changes in physical function and health status resulting from injury, diseases or other causes.

· Assesses patient ability to safely and effectively perform functional mobility skills and evaluate muscle, nerve, joint, and integumentary systems

· Designs and implements a treatment plan of care utilizing practice standards and recommended techniques and modalities, insurance guidelines, and patient’s needs. Monitors patient’s response and progress to treatment and modifies treatment plan as appropriate

· Documents and charges accurately, completely, and timely using measurable and objective terminology, following standards of practice, insurance guidelines and department/hospital policies and procedures

· Provides discharge planning assistance and information regarding appropriate selection/use of adaptive equipment and follow up services

· Communicates appropriately and effectively with patients, family and healthcare professionals within and across care settings and communities



*The expertise and experiences you’ll need to succeed: *

*Minimum qualifications*:
* Bachelors degree, Masters degree or Doctorate degree in Physical Therapy
* Must retain a valid drivers license and current automobile insurance, as driving between facilities is a requirement of this position.
* Current Florida Physical Therapy license required.
* *Preferred qualifications:*
* Entry level competency of physical therapy practice, theory, principles and applications as defined in competency/department policies

Pay: $37.30 - $55.95 per hour
