NURSE/LPNResponsible for providing specialized nursing services to patients and caregivers at home or in alternative settings. Provides technical nursing and skilled interventions as ordered by the physician. Instructs, motivates, and assists patients / caregivers in learning and improving ability to live independently at home.

Job Duties:Ensures physician orders are followed and included in patients plan of careDevelopment and revision of the plan of care based on physician ordersEnsures nursing orders are carried out in a professional manner satisfying professional ethics, patient rights and State and Federal codes and regulationsContinually monitoring patient outcomes and reporting changes to supervisors to prevent hospital readmissionParticipates in effective communication amongst other clinicians providing care to the patientCompletes and submits all required documentation within established agency guidelinesCommunicates the patient's progress to supervisor and make changes as necessary under the physician's directionCovers after hours, weekend and holiday on call to meet the branches needMaintains confidentiality of all company and patient related information. Protects patient rights as defined in the patient Bill of RightsAttends and participates in staff meetings, required in-services and other meetings as requestedInstructs patients/caregivers of the safety measures appropriate to care and environment and observes/implements standard precautions and infection control for all patients.Ensures accurate patient assessments are completed timelyCoordination of related services for patientsResponse to emergency patient needs timely and escalating to supervisors and physicians as neededAlways represents self in a professional and ethical mannerTakes professional responsibility for self-protection during required work in potentially hazardous situationsCompletes all required patient care documentation and reports accurately and in a timely mannerCommunicates with clinical staff, physicians, and other health care providers to assure the quality and continuity of patient careOther duties as assigned.

Requirements:

Minimum Job Qualifications:Associate degree in nursing or equivalentCurrent and unrestricted LPN clinical licenseTwo (2) years of clinical nursing experience is requiredFive (5) years of related HME experience is preferredValid and unrestricted driver's license
