The Physical Therapist will provide exceptional physical therapy services to patients, effectively communicating progress with interdisciplinary teams. Key responsibilities include:Delivering Physical Therapy services including assessment, evaluation, and the implementation of procedures, setting mutual goals, and conducting teaching and training activities as outlined in the patient's Plan of Care.Updating care plans continually as changes occur or upon recertification.Submitting detailed Physical Therapist visit notes to Clinical Supervisor as required.Conducting OASIS time point assessments and participating in discharge planning processes according to Medicare criteria, policies, and procedures.Participating in staff meetings, team conferences, and required educational in-services.Carrying out additional duties assigned by the Administrator.Applying the 4Ms (What Matters to the patient, Medications, Mentation, and Mobility) to ensure Age-Friendly Care.Qualifications
To be successful in this role, candidates must meet the following qualifications:A current, valid State Physical Therapy License in the state of practice.A valid driver's license, a clean driving record, proof of insurance, and reliable transportation.At least one year of experience as a home care Physical Therapist, with proficiency in comprehensive assessments and OASIS-C preferred.Ability to exercise sound professional clinical judgments.Skilled in assessing and documenting patient needs and formulating customized care plans.Proficient in essential clinical skills.Competency in Microsoft Office applications.Willingness to drive 2-4 hours a day.Pay Transparency
