Examples of Duties
Job PurposeUnder general supervision, plans, coordinates, and provides professional public health nursing assessment and person-centered care planning to individuals, families, and groups in home and community-based settings.
Essential Job DutiesProvides nursing assessments, person-centered care planning and referral services to individuals, families, groups, and institutions.Review Maryland Department of Health Long Term Services and Supports Tracking System (LTSS) assessment requests upon assignment of cases.Contact assigned clients/representatives timely.Conducts home visits.Complete nursing assessments and high-quality person-centered care planning timely to ensure timely access to home and community-based services.Provides case management, ongoing monitoring and evaluation of person-centered care plan.Assesses and monitors clients' physical and mental states of health and determines need for community-based resources.Ensures compliance with state, federal and Medicaid regulations.Provides health screening education and counseling in the areas of disease prevention, health maintenance, and mental health.Participates in the instruction and orientation of new public health nurses.Prepares and maintains nursing records and reports.Examples of Other DutiesPerforms other related duties as required.Qualifications

Licenses and Certificates
Possession of a valid license to practice as a professional registered nurse, issued by the Maryland State Board of Examiners of Nurses, is required at the time of appointment. Possession of a valid driver's license equivalent to a noncommercial, class C Maryland driver's license is required at the time of appointment.

Paid LeaveSick Leave
- Monthly sick leave earnings prorated based on the number of hours worked per week. No maximum on accrual. Accrued balance can be applied toward service credit at retirement.
Vacation Leave
- Monthly vacation leave earnings prorated based on the number of hours worked per week. Earnings increase after five, ten and fifteen years of service generally.
Holidays
- Part-time 30 to 39 hour non-merit or non-classified employees will generally be paid for ten holidays per calendar year. Other part-time, non-merit or non-classified employees working 29 hours per week or less will receive two paid holidays per calendar year. These holidays are the Christmas holiday and the Independence Day holiday.
