Job Description:

- Provide physical therapy services in designated territories, including San Jose, Santa Clara, Sunnyvale, Cupertino, Milpitas, Mountain View, Campbell, Los Altos, Saratoga, Los Gatos.- Additional travel outside assigned areas may be required.- Must be willing to see Covid positive and persons under investigation (PUI) patients.- Weekend coverage may be required.- Maintain productivity with at least 27 points per week.- Travel outside the area or work weekends as required to meet weekly visit expectations.- Participation in patient evaluations, discharges, follow-ups, and relevant documentation.- Hours are approved on a per-visit basis, with specific point allocations for different visit types.- All tasks, including travel, visit, documentation, and patient communication, are covered in approved hours.- Ensure compliance with all credentials, including BLS and state licensing requirements.- All requested time off must be disclosed prior to confirmation.

Note: Holiday coverage is required, and billing rules apply as stated per contract guidelines.
