We believe in a personal touch, offering holistic support to our patients and their families during their most challenging times.Role Overview:As a Nurse Practitioner in our Advanced Illness Management (AIM) Program, you will play a crucial role in facilitating palliative care and ensuring patients receive individualized treatment tailored to their unique needs. Your expertise will help create comprehensive care plans while working collaboratively with a team of healthcare professionals.Key Responsibilities:Collaborate with physicians and the palliative care team to assess patients and provide diagnosis, treatment, and follow-up care.Manage the entire process of advanced illness management from assessment through discharge, focusing on changes in health status, pain control, and overall patient and family wellbeing.Develop personalized plans of care in coordination with patients, families, and the healthcare team.Actively participate in evening and weekend on-call services as needed.Prescribe medications responsibly and order necessary diagnostic tests while evaluating their effectiveness regularly.Maintain communication with healthcare team members and document findings clearly in patient records to support best practices.Stay informed about Palliative Medicine Quality Measures and participate in documentation and reporting processes.Qualifications:Education & Experience:Master's degree in Nursing from an accredited institution or an equivalent as outlined by state law.Minimum two years of experience as a Nurse Practitioner.Preferred specialties include Family or Adult Medicine, and knowledge of Value-Based Care is a plus.Skills & Licenses:Strong leadership and communication skills, both written and verbal.Ability to make independent decisions and work efficiently with electronic medical records.Current RN license, Nurse Practitioner certification, and DEA licenses as required by state law.What We Offer:We provide a competitive compensation package including:Comprehensive medical, dental, and vision plans.Generous paid time off and holidays.401(k) plan with company matching contributions.Career growth opportunities across our branches.Tuition reimbursement and access to continuing education.Supportive work environment with a focus on personal and professional development.Join Us!
