A dedicated Physical Therapist is sought to join the Jet Health team serving patients in Westminster, Arvada, Thornton, Wheat Ridge, Northglenn, CO and surrounding areas.

Job Summary: This role involves providing coordinated care to patients/clients of all age groups, planning and implementing individualized care plans to restore or maintain patient well-being.

Key Responsibilities:

  • Develop and Implement Treatment Plans: Create customized physical therapy treatment plans based on medical referrals and evaluations.
  • Evaluate and Document Progress: Assess patient/client progress, adjusting treatments as necessary.
  • Coordinate Care: Collaborate with other healthcare professionals to ensure comprehensive treatment.
  • Patient Education: Educate patients/clients and their families on treatment plans, home exercises, and preventative care.
  • Record Keeping: Maintain accurate and timely patient records in compliance with company and regulatory standards.
  • Durable Medical Equipment: Utilize equipment as needed and provide training to patients/clients.
  • Ensure Patient Safety: Prioritize patient comfort and safety during therapy sessions.

Requirements:

  • Education: Bachelor of Science in Physical Therapy from an American Physical Therapy Association approved program.
  • Licensure: Current State License or a multi-state license issued by a state member of the Physical Therapist Licensure Compact (PTLC).
  • Experience: At least one year of experience as a Physical Therapist in an acute care, rehabilitation setting, or home health preferred.
  • Skill Set: Proficiency in therapy skills, strong interpersonal skills, current CPR certification, and knowledge of durable medical equipment.

Work Environment:

  • Work in healthcare facilities or patients'/clients' homes in various conditions.
  • Possible exposure to blood, bodily fluids, and infectious diseases.
  • Ability to work a flexible schedule and travel locally.
