JOB DESCRIPTION
POSITION: QI COORDINATOR
DEPARTMENT: Medical Unit
STATUS: Full-Time, Non-Exempt
POSITION SUMMARY: Under the direction of the Supervisor of Chief Medical
Officer. The QI Coordinator offers assistance to the
providers for necessary patient care services within the
scope of the clinic.
Essential Duties and Responsibilities:
1. Provides support in the organization's quality improvement efforts and performance
improvement activities which includes: data monitoring, and facilitation of quality
improvement projects.
2. Works with key stakeholders from the clinical care team to improve our performance
on various UDS, HEDIS, and other metrics/quality indicators.
3. Serves as the quality improvement team lead and is responsible for communicating
progress of quality improvement projects to the clinical care team.
4. Collects and analyzes data to identify opportunities, and/or barriers.
5. Works alongside key stakeholders from the clinical care team to implement
interventions clinic-wide that address problem areas.
6. Tracks, measures, and maintains comprehensive project documentation for all
assigned projects and submits regular progress reports to the Director of Infection
Prevention
7. Tracks certain diagnostic tests and referrals relating to metrics/quality indicators;
including contacting other clinics for records and locating past medical records to enter
them correctly in the electronic health record.
8. Travel to clinics and sites of T.H.E to participate in various Quality Improvement
activities and meetings.
9. Protects patients' rights by maintaining strictest confidentiality of personal and
financial information; adhering to all HIPAA guidelines/regulations.
10. Maintains operations by following policies and procedures; reporting needed
changes.
11. Assist in maintaining office and building fire/safety regulations.
12. Ensures clinic flow by being an active team member.
13. Practice active listening, empathy and cultural humility with patients to schedule
appointments and address challenges of patients at-risk of missing appointments.
14. Outreach to patients and assist patients with accessing additional resources and/or
clinical services, including transportation and interpretation services.
15. Coordinate appointments with health care providers to ensure timely delivery of
diagnostic and treatment.
Education and Qualifications
• High School Diploma or equivalent
• Certification in CPR

• Minimum one year of QI experience
• Ability to work with people of diverse cultural, educational, social, and economic
backgrounds.
• Ability to work independently and a self-starter with minimal supervision
• Excellent oral and written communication skills
• Excellent organizational skills
• Computer proficiency
• Experience utilizing an Electronic Medical Records (EMR) system is highly
desirable
• Bilingual preferred
