We are seeking a dedicated and compassionate Palliative Care Nurse Practitioner (NP) to join our client’s interdisciplinary team, providing holistic care to patients with serious or life-limiting illnesses. The ideal candidate will have experience in palliative care or hospice, demonstrating expertise in symptom management, emotional support, and end-of-life care planning. This role involves working collaboratively with physicians, nurses, social workers, and other team members to ensure the highest quality of patient-centered care.
*Key Responsibilities:*
* Perform comprehensive assessments of patients with chronic, advanced, or terminal illnesses, including physical, emotional, social, and spiritual needs.
* Develop and implement individualized care plans focused on symptom management, quality of life, and patient goals.
* Provide pain and symptom management, including prescribing medications and treatments in accordance with state regulations.
* Facilitate discussions about advance care planning, goals of care, and end-of-life decisions with patients and families.
* Collaborate with the interdisciplinary palliative care team to ensure a coordinated approach to patient care.
* Serve as a resource and consultant to other healthcare providers regarding palliative care principles and practices.
* Provide education and emotional support to patients' families and caregivers, helping them navigate the complexities of serious illness and end-of-life care.
* Act as a liaison between patients, families, and the healthcare team to address concerns and ensure understanding of care plans.
* Educate patients, families, and staff on palliative care principles, including symptom management and advance care planning.
* Advocate for patients' needs and preferences within the healthcare system.
*Requirements:*
* Master’s degree in Nursing with certification as a Nurse Practitioner (NP).
* Current NP licensure in New York.
* Minimum of 1 year of experience in palliative care or hospice.
* Spanish-speaking abilities preferred but not required.
* Excellent communication and interpersonal skills, with the ability to have sensitive conversations about serious illness and end-of-life care.
* Strong clinical judgment and critical thinking skills.
* Ability to work collaboratively within a team and independently when needed.
* Empathy, compassion, and a commitment to patient-centered care.

Actual compensation offered to the successful candidate may vary from posted hiring range based upon geographic location, work experience, education, and/or skill level, among other things. Details about eligibility for bonus compensation (if applicable) will be finalized at the time of offer._
