*Position*-RN Case Manager

*Location*- Tuba City, AZ 86045

*Shift* - Monday to Friday, 8am-5pm & Weekend rotation with call

*Duration*- 13 weeks

*Note* - 50 miles radius

*Weekly Pay -(above 50 miles) *

W2 Pay Rate RT (Regular Time) -$41/hr + Nontaxable Stipend - $1560/week - Weekly gross - $3200/week for 40hrs

*REQUIRED QUALIFICATIONS – *

Must have a minimum of one (1) year of experience in the clinical care setting

Experience and knowledge of discharge planning, case management, or utilization review processes.

Demonstrate knowledge of electronic health record systemLicenses and/or Certifications

Associates degree in Nursing, must obtain bachelor’s in nursing within 2 years of hire.

*Responsibilities*
* Conduct comprehensive patient assessments to develop and implement individualized care plans.
* Coordinate discharge planning to ensure smooth transitions from hospital to home or other care settings.
* Collaborate with healthcare teams to provide managed care and optimize patient services.
* Maintain compliance with HIPAA regulations and utilize medical terminology effectively.
* Provide education and support to patients and families regarding health management and resources.
* Monitor patient progress and adjust care plans as necessary to meet evolving needs.
