Job Title: Post-Acute Long-Term Care Nurse Navigator

The Staff Pad has partnered with a client in Montana who aims to be the gold standard for healthcare. We are seeking a skilled Post-Acute Long-Term Care Nurse Navigator to support this vision.

Responsibilities:

• Advocate for patients and facilitate their journey through the healthcare continuum, enhancing satisfaction and outcomes.


• Regularly visit local skilled nursing facilities (SNFs) to ensure continuity of care and build strong relationships with care teams.


• Collaborate with multidisciplinary teams to create and implement individualized care plans.


• Coordinate outpatient and virtual visits, ensuring timely scheduling of appointments and diagnostic procedures.


• Maintain accurate documentation in patient electronic medical records (EMRs) and manage communication between SNF staff and providers.


• Educate and support SNF staff on standard operations and care-related topics.


• Identify improvement opportunities within care processes and implement strategies to enhance patient experiences.

Requirements and Qualifications:

• Minimum of 3-5 years of nursing experience, with at least one year in acute care preferred.


• Graduate of an accredited nursing school; Bachelor of Science in Nursing (BSN) preferred.


• Current Montana Registered Nurse (RN) license and Basic Life Support (BLS) certification required.


• Excellent communication, time management, and collaboration skills.
