Job Title: Post-Acute Long-Term Care Nurse Navigator
The Staff Pad has partnered with a client in Montana who aims to be the gold standard for healthcare. We are seeking a skilled Post-Acute Long-Term Care Nurse Navigator to support this vision.
Responsibilities:
- Advocate for patients and facilitate their journey through the healthcare continuum, enhancing satisfaction and outcomes.
- Regularly visit local skilled nursing facilities (SNFs) to ensure continuity of care and build strong relationships with care teams.
- Collaborate with multidisciplinary teams to create and implement individualized care plans.
- Coordinate outpatient and virtual visits, ensuring timely scheduling of appointments and diagnostic procedures.
- Maintain accurate documentation in patient electronic medical records (EMRs) and manage communication between SNF staff and providers.
- Educate and support SNF staff on standard operations and care-related topics.
- Identify improvement opportunities within care processes and implement strategies to enhance patient experiences.
Requirements and Qualifications:
- Minimum of 3-5 years of nursing experience, with at least one year in acute care preferred.
- Graduate of an accredited nursing school; Bachelor of Science in Nursing (BSN) preferred.
- Current Montana Registered Nurse (RN) license and Basic Life Support (BLS) certification required.
- Excellent communication, time management, and collaboration skills.
