Job Description
  • Coverage Needed:
    • March 14, 2025 - June 2025
    • Monday- Wednesday-Friday, 8am-5pm
  • Clinical Information:
    • Patient Volume: 120
    • Support Staff: Medical Assistants and Registered Nurses
    • Will work IP and OT
    • This is for both inpatient (emergent devices, Watchman, and Devie clinic checks)
  • Required:
    • CV must be in Month / Year format
    • Must have license in hand at time of submission, or proof that the license application has been submitted
    • Any gaps greater than 30 days will require an explanation on the CV
    • DEA must remain in state to maintain privileges. Provider must be willing transfer and maintain DEA in state for the duration of privileges being active, or obtain a new in state DEA to remain in state for the duration of privileges being active
    • Current Flu Vaccine
    • Must be fully vaccinated for Covid 19
      • Exemptions only allowed for religious or medical reasons, once approved by the facility.
    • Active IL License and DEA (or easily transferrable one)
    • Board Certified
