Facility: A 13-bed critical access hospital and a 34-bed licensed basic care facility.

Opportunity:

• Over 800 referrals from facility per year
• Shifts: Provide outpatient services monthly. 3-4 consecutive days/month. Ideally, surgery is
• day 1 and clinic the following days for monitoring and follow-up on surgery patients
