SNF and Clinic Coverage needed in Vermon

Locum needed starting as soon as credentialed
Preventative visits, acute needs and chronic ongoing needs.
SNF admissions, discharges, acute needs and periodic assessments.
1.5-2 days in clinic seeing patients 65 and up, and 3-3.5 days in the skilled Nursing Home.
No procedures
MA support and RN triage support along with MDs and APPs on the team
8-12 patients per day at SNF and up to 16 patients in 8 hrs in clinic
SNF uses PCC and EPIC/ clinic is EPIC only

Offering $170/hr or more depending on experience and qualifications
