Key Responsibilities:

• Lead case management teams with expertise in Case Management and RN practices.
• Develop and implement patient-centered care plans to ensure optimal health outcomes.
• Collaborate with healthcare professionals to coordinate seamless patient transitions.

Work Details:

• 13 weeks contract duration.
• 40 hours per week , including flexible scheduling to meet the needs of patients and staff.
• 8 hours, days shift schedule.
• Travel nursing assignment type.
