• Providing high-quality patient care and management services

• Monitoring and evaluating patient progress, making adjustments to care plans as needed

Requirements:

• Current licensure as a Registered Nurse (RN)
• Experience in case management or a related field
• Strong clinical skills and knowledge of case management principles and practices
• Excellent communication and interpersonal skills
• Ability to work independently and collaboratively as part of a team
