Job Title: Nurse Practitioner - Palliative Care
Gentiva is seeking a compassionate and skilled Nurse Practitioner to provide palliative care services to patients in need.
Key Responsibilities:
- Collaborate with healthcare teams to deliver advanced illness management and palliative care services to patients, ensuring a comprehensive and individualized treatment plan.
- Assess patients' physical, emotional, and social needs, and develop personalized care plans to address these needs.
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
- Prescribe medications, order diagnostic tests, and provide treatments as necessary, within the scope of professional practice.
- Participate in evening/weekend call as required, providing on-call services in a clinically competent and responsive manner.
Requirements:
- Master's degree in Nursing from an accredited school of nursing.
- Current licensure as a registered nurse or equivalent as required by state law for Nurse Practitioner licensure without board actions.
- Current Nurse Practitioner certification/license without board actions.
- Collaborative agreement with a supervising physician, that is part of the Professional Services Corporation, that is agreeable to supervise work in advanced illness management and palliative care program, as required by state law.
- Part B Provider certification.
- Federal DEA license.
- State DEA license (if required).
- ACHPN – Certification in Hospice and Palliative Care Preferred.

Compensation may vary within the salary range provided based on several factors including but not limited to a candidate's location, experience, education, skills, licensure, certifications and department equity. Gentiva provides associates with a comprehensive benefits and total rewards package, of which base pay is just one piece.
Salary Range – $96,390-$120,490
