Minimum of 1-year experience working as a Social Worker in the healthcare field required. No Uber in Alliance. 8am-5pm Assists families during the course of clinic visits, ER visits, and hospitalizations as the patient s advocate and guides them through health diagnosis and illnesses. Social Worker will assist families with the social, emotional, financial, and psychological concerns associated with having a loved one in the hospital. Social Worker will conduct psychosocial assessments on patients and determine the need for resources. Social Worker can also act as a liaison between the patient, family, health care team, and community agencies. Social worker will organize crisis intervention for patients and family members as well as provide counsel to patient and families.
