Job Description

As a Registered Nurse (Home Health) professional, you will be responsible for providing high-quality patient care and support services to individuals in their homes. Your primary focus will be on delivering comprehensive nursing care that addresses physical, emotional, and social needs of patients.

Key Responsibilities:

• Conduct thorough assessments of patients' health status and develop personalized care plans to meet their unique needs.
• Administer medications, perform wound care, and provide other interventions as necessary to promote patient well-being.
• Collaborate with healthcare teams, family members, and caregivers to ensure seamless transitions and continuity of care.
• Maintain accurate and detailed records of patient interactions, treatments, and outcomes.

Requirements

To be successful in this role, you must possess:

• At least 1 year of full-time experience as a Registered Nurse (Home Health) within the last 2 years.
• A valid state and/or national nursing license.
