Position Overview:
We are seeking Licensed Social Workers (LSW) and Licensed Independent Social Workers (LISW) to join our team as Therapist/Behavioral Health Care Managers . In this role, you will provide psychotherapy, group therapy, and collaborative care management to residents in need.
Key Responsibilities:

  • Deliver individual and group psychotherapy sessions tailored to the needs of residents.
  • Facilitate collaborative care management by coordinating with psychiatrists, primary care providers, and other team members.
  • Address the mental health needs of residents with chronic severe mental illness, substance use disorders, depression, and major neurocognitive disorders.
  • Develop and implement therapeutic interventions using evidence-based modalities.
  • Maintain detailed and accurate clinical documentation in an electronic medical records (EMR) system.

Qualifications:
  • Current LSW or LISW licensure in OH.
  • Master's Degree in Social Work from an accredited institution preferred but not required.
  • Experience working with diverse patient populations, including those with chronic severe mental illness and major neurocognitive disorders.
  • Proficiency in using electronic medical records (EMR) systems.
  • Familiarity with evidence-based therapeutic modalities (preferred).
  • Strong interpersonal and communication skills.
  • Ability to work collaboratively within a multidisciplinary team.
